38 weeks pregnant and having STRONG cramping in my upper abdomen and lower back pain.?

This is my 4th child and I have never felt this before! Could I be in labor? I called my doc (who is a complete quack) and she told me to bear through it unless my water breaks or I have regular contractions. At this point Im worried something may be wrong!
